What is the BridgingCare Planner (BCP)?
The BCP is a patient registry provided by Health Management Innovations at GlaxoSmithKline. The BCP is unique in that it was designed to be a patient registry, providing the flexibility to track patients with multiple chronic diseases such as diabetes, asthma, and heart failure while proactively managing treatment plans at the point-of-care. The BridgingCare Planner is licensed to physicians by GlaxoSmithKline as part of a research collaboration.
